Consumer Health Record Access & Complaints
Texas HB 4224 – Effective September 1, 2025
In accordance with Texas Health and Safety Code Chapter 181 and House Bill 4224 (89th Regular Session), patients have the right to access their health care records and to receive information on how to submit questions or complaints.
Requesting Your Health Care Records
Patients may request copies of their medical records from our office using any of the following methods:

Please include the following with your request:

Patient full legal name and date of birth

Description of records requested (or indicate “all records”)

Copy of a valid photo ID

Signature of the patient or legally authorized representative
Requests are processed in accordance with Texas law. Reasonable, legally permitted fees may apply.
